Some of these are discussed as follows:

                   Closed-ended questionnaire: Closed-ended questions provide predefined
answer options. The person answering selects the most appropriate answer
from the given choices. This type is useful for collecting structured and easy-to-

                     analyse data.

                     Example:

What is your preferred mode of transport?

                     ☐ Car ☐ Bus ☐ Train ☐ Bicycle

 Open-ended questionnaire: Open-ended questions allow the person answering
to write their own response. These questions help gather detailed opinions, ideas
                     and explanations.

                     Example:

What improvements would you suggest for public transport?

 Multiple choice questionnaire: The person answering is given multiple options,
usually with one or more correct answers. It is widely used for surveys and

                     assessments.

                     Example:

Which of the following do you use regularly?

                     a) Mobile phone b) Laptop

                     c) Tablet d) Desktop

 Yes/No questionnaire: This type includes binary questions with only two possible
answers: Yes or No. It is useful for collecting clear and direct responses from the
                     person answering.

                     Example:

Do you use online services regularly?

                     ☐ Yes ☐ No

 Rating scale questionnaire: The person answering ethical minds
rates their opinion or experience on a scale (such

as numbers or levels). It helps measure attitudes, AI systems learn from data
satisfaction or preferences. to make decisions. If the

Example: data is unfair, their decisions
                                                                                       can also be unfair.
How satisfied are you with the service?
