CREATING A FORM
The steps to create a form are as follows:
1 Enter the title of the form.
2 Click on the drop-down list to see
the categories of question.
3 Select the type of question you want to create.
4 Click inside the Untitled Question text
box and type a new question.
5 Click on the Add question button
on the side toolbar.
